近期，疫情防控形势复杂，为保护出入境人员生命安全和身体健康，请来中心办理业务的客人及陪同人员持绿码，在进入中心业务场所前完成“健康申报卡”，以便查验。请根据《广州市新型冠状肺炎疫情防控指挥部关于进一步加强来（返）穗人员健康管理工作的通告（第35号）》要求配合做好疫情防控工作。如您处于隔离观察期、健康监测期，或途经境内疫情高风险地区未满7天，“健康码”为“黄码”或“红码”的，请暂勿前来本中心办理业务。有发热、干咳等不适症状的，请暂缓前来办理业务，在做好个人防护的同时，尽快前往发热门诊进行排查。
在遵守以上疫情防控措施要求的同时，请注意以下事项：
1、入境人员，请根据政府要求严格落实健康监测闭环管理措施，已解除集中隔离或居家隔离等管理措施后，出示“健康码”，携带体检预约二维码、解除隔离证明和核酸检测报告的原件及复印件前来办理业务。
2、已长期在境内且未经隔离的入境人员，请配合出示护照入境记录及“健康码”。
3、进入本中心请携带有效身份证件、出示“健康码”、佩戴口罩并配合信息登记及测量体温。
4、为避免交叉感染风险，尽量减少陪同人员进入业务场所。

According to the latest COVID-19 situation and the arrangements by the Guangzhou Municipal Government, to protect the safety and health of entry-exit personnel. “Green code”and “Health declaration code” will be checked before you enter our center.If you are kept in the quarantine or in a health monitoring period, or have been to the high risk areas within 7 days, or had a red/yellow “health code”, please do not come to our center for physical examination.
If you have fever, dry cough and other symptoms, please do not come to our center, go to the fever outpatient department/fever clinic immediately with proper self protection.
The relevant precautions are hereby notified as follows：
1. When you entry China, Please strictly observe the quarantine regulations, after you were released from quarantine(5-days) and completed the health monitoring(3-days), bring the “health code”, the code for medical examination appointment, the certificate of quarantine and the last report of Covid-19(originals and copies) when you come to our center.
2. For foreigners who have been in China for a long time and have not been quarantined, please cooperate to show your passport entry records and "health code".
3. To enter our center, please bring a valid ID, show your "health code", wear a mask, and cooperate with information registration and temperature measurement.
4. To avoid the risk of cross-infection, please try to minimize the number of accompanying persons entering our center.
